- Please submit a copy of your Baptism Certificate with this form if you have been baptized in any denomination
- Later on you will need to have a Sponsor or Godparent. We will discuss this further in class.
- You might need an annulment, if you have been divorced or remarried. We will discuss this also at our first meeting.
